From the moment of admission to Hannah’s House, we take great care to craft an individualized continuing care plan for each woman’s eventual discharge. In order to develop a comprehensive plan for lifelong recovery management, we consult not only with the patient herself but with her family and trusted professional referents. This approach ensures that we provide each woman with a customized roadmap for success.

What is a Continuous Care Plan?

A whole new kind of recovery work begins when residential treatment ends. Often known as “discharge planning,” continuous care planning is an integral component of treatment that helps the entire family system navigate the ongoing challenges of recovery for a lifetime. A detailed and organized continuous care plan provides stability for the person seeking recovery and peace of mind for her loved ones.

When does the continuous care planning process start at Hannah’s House?

At the onset of treatment, the Alumni Care Team at Hannah’s House works side by side with each patient’s multidisciplinary team to create a comprehensive plan for the lifelong healing journey.

Who handles continuing care programming at Hannah’s House?

Continuous care planning is a collaborative effort, and each patient works alongside her personal Alumni Care Coordinator to develop a plan in line with her unique strengths and challenges. These trained addiction specialists coordinate with the patient’s Primary Therapist, as well as auxiliary points of contact to develop strategies that take all aspects of recovery into account. Hannah’s House coordinators attend our multidisciplinary team meetings to better understand each patient’s background and clinical needs.

Who does Hannah’s House involve in the planning process?

Most women in our program sign consent forms that allow our staff to connect with their loved ones and professional referents. These trusted and important contacts are organically included in the planning process through ongoing communication with our team, and they often provide valuable insight into the needs of the family and other important relationships.

Key Components of a Continuing Care Plan

The individual needs of every Hannah’s House patient are always considered as our staff develops a continuing care plan that includes tailored recommendations in these areas:

Counseling

As a team, we coordinate with trusted professional referents to guide our patients to top-tier clinical services that address the natural ups and downs of recovery. Ongoing counseling is a core component for most patients. Recommendations may include individual or family therapy or varying levels of clinical support, such as Intensive Outpatient Programming or Outpatient Services. These services provide support for the natural challenges that arise during life after treatment and pave the way for healthy living.

Medication

Co-occurring disorders or “dual diagnoses” are common with addiction. After they leave our care, many patients of Hannah’s House will need to adhere to their ongoing medication recommendations, including when and how to take any medications prescribed to them. We’ll connect our patients with local mental health professionals and psychiatrists who can monitor their ongoing progress with medications.

Family Programming and Support

Addiction is a family disease, and studies show that outcomes improve when a patient’s loved ones work on their own self-care and recovery. We offer support to the entire family through our intensive Family Programming. This program takes every family member’s needs into account and may involve individual, family, or marriage counseling, as well as supportive fellowships for family members (such as Al-Anon or Alateen). By identifying new and existing support systems, each member of the family system is empowered to heal from dysfunctional dynamics and prevent the return to old habits that keep addiction alive.

Alumni Care

Alumni Care at Hannah’s House begins at admission, carries throughout treatment, and lasts a lifetime. Our team utilizes recovery support and 12-Step programming to enhance life-improving practices for wellness. These fellowships provide the motivation and guidance necessary to maintain lifelong recovery. We offer comprehensive Alumni Care, including ongoing phone support, meetings, workshops, and events. Our smartphone app provides yet another touch point for Hannah’s House patients after they discharge.

Accountability and Structure

Leaving the intensive, well-structured residential treatment environment without a concept of how to spend one’s time afterward can unnecessarily set a woman in recovery up for failure. With the assistance of their primary therapist, our patients develop a working daily schedule, which includes both clinical and medical recommendations, as well as other sober activities such as attending work/school, volunteering, engaging in a 12-Step program, or exercising. All of these activities can provide much-needed accountability and structure

Safe Living Accommodations

For many of our patients, returning to an unhealthy living arrangement can be disastrous. A comfortable, safe living environment upon discharge supports ongoing recovery. As each family system approaches the continuing care process, our team will work to ensure that safe living accommodations are available immediately following discharge. Sometimes, the care plan may also include a step-down treatment program, such as partial hospitalization (PHP) or transitional living.

The Goal of the Hannah’s House Alumni Care Team

The most successful outcomes occur when the patient develops healthy relationships and interests that do not end at graduation from our care. Ultimately, sobriety requires the adoption of new thought patterns and the application of recovery principles to everyday life. Through ongoing collaboration with our team, the entire woman’s family is able to discover a plan that supports freedom from substance use — not only for today but for a lifetime.
